DOT SAP Process

• SAP Process fee is $545, paid in full at the time of service.

• Employee and SAP will meet face to face remotely or in office for the initial and final evaluation which can take up to 60 minutes each.

• After completed education or treatment, the employee and SAP will discuss return to duty eligibility and any further steps in the final face to face meeting.

 

Additional Fees:

• Employee must take a urine drug/alcohol screen at minimum, after the first appointment. A list of recommendations will be provided.

• DOT requires that the employee attend an educational program or treatment program based on the employee's needs and situation.

• The SAP cannot determine if the employee needs education or treatment until the evaluation and urine screening are completed.

 

Other Information:

• Education can take 1- 30+ days to complete. This depends on the employee’s plan, needs, and motivation to complete the education.

• Education fees can vary depending on the program needed from $60.00- $150.00 +. Treatment costs vary depending on the provider’s fees. The SAP and Employee will discuss which service best fits the employee’s needs.

• Treatment can take 30+ days. This depends on the employee’s plan and needs.

• Urine screens vary in cost from $50.00- $150.00, depending on the substances being tested.
