

DOT SAP Process
Easy as 1,2,3! Steps and approximate cost of the SAP Process
Final cost and time line depends on the individual case.

SAP Process fee is $545, paid in full at the time of service.

Employee and SAP will meet face to face remotely or in office for the initial and final evaluation which can take up to 60 minutes each.

After completed education or treatment, the employee and SAP will discuss return to duty eligibility and any further steps in the final face to face meeting.
Additional Fees:

Employee must take a urine drug/alcohol screen at minimum, after the first appointment. A list of recommendations will be provided.

DOT requires that the employee attend an educational program or treatment program based on the employee's needs and situation.

The SAP cannot determine if the employee needs education or treatment until the evaluation and urine screening are completed.
Other Information:

Education can take 1- 30+ days to complete. This depends on the employee’s plan, needs, and motivation to complete the education.

Education fees can vary depending on the program needed from $60.00- $150.00 +. Treatment costs vary depending on the provider’s fees. The SAP and Employee will discuss which service best fits the employee’s needs.

Treatment can take 30+ days. This depends on the employee’s plan and needs.

Urine screens vary in cost from $50.00- $150.00, depending on the substances being tested.
